Annual dues, required for membership and participation in club activities, are only $60.00 (if paid by check) and $62.50 if paying online for the 2025-2026 club year. Club meetings and activities start in September. Please make checks payable to Forest Creek Woman's Club and drop off  at 3304 San Simeon Way or pay online using our secure payment system (available at the end of the application form). If you have questions, please contact membership.

Liability Waiver: I understand that the Forest Creek Woman's Club and other persons acting as its agents, representatives and sponsors will not be responsible for any claim for damages, compensation, or otherwise on my part, my family and my agents, growing out of or resulting from injury in connection with participation in the Forest Creek Woman's Club activities. Submission of this form electronically equals the participants signature.

Photo Release: I grant to Forest Creek Woman's Club and its representatives the right to take photographs of me and my property in connection with club activities. I authorize Forest Creek Woman's Club, its assignees and transferees to copyright, use and publish the same in print and/or electronically. I agree that Forest Creek Woman's Club may use such photographs of me with or without my name and for any lawful purpose, including for example, such purposes as publicity, illustration and web content.
